PCOS (Polycystic Ovary Syndrome)

Affects 8-13% of women. Diagnosis requires 2 of 3 Rotterdam criteria:

  1. Oligo- or anovulation (irregular or absent periods)
  2. Clinical and/or biochemical hyperandrogenism (acne, hirsutism, elevated testosterone)
  3. Polycystic ovarian morphology on ultrasound (≥12 follicles per ovary or volume >10 mL)

Endometriosis Staging

StageDescriptionFertility ImpactTreatment
Stage I (Minimal)Few superficial implantsMild reductionExpectant management, IUI, or IVF
Stage II (Mild)More implants, deeperModerate reductionLaparoscopic excision + IUI/IVF
Stage III (Moderate)Endometriomas, adhesionsSignificant reductionSurgery then IVF preferred
Stage IV (Severe)Large endometriomas, dense adhesionsSeverely reducedIVF recommended

Ovarian Reserve Testing

AMH (Anti-Mullerian Hormone): Best single marker of egg quantity. Normal: 1.0-3.5 ng/mL. Low: <1.0 ng/mL. Does NOT predict egg quality.

AFC (Antral Follicle Count): Ultrasound count of 2-10mm follicles on Day 2-4. Normal: 10-20. Low: <6 total.

FSH (Day 3): Normal: <10 IU/L. Elevated: >10 IU/L suggests diminished reserve.
