Sleep & Safety
AAP safe sleep guidelines, SIDS prevention, sleep training methods with evidence and outcomes, wake windows by age, and sleep regression guide.
Safe Sleep Rules (ABCs)
- Alone: Baby sleeps alone in their own sleep space. No bed-sharing.
- Back: Always on back for every sleep. Once baby rolls both ways independently (~6 months), they can choose position.
- Crib: Firm, flat surface. Fitted sheet only. No blankets, pillows, bumpers, stuffed animals.
SIDS Prevention
- Back to sleep for every sleep
- Firm, flat surface meeting CPSC standards
- Room-share for at least 6 months, ideally 12 months
- No overheating (68-72F, one layer more than you)
- Offer pacifier at sleep (after breastfeeding established)
- No smoking exposure
- Breastfeeding reduces SIDS risk
Sleep Training Methods
| Method | Age | Timeline | Evidence |
| Ferber (Graduated Extinction) | 4-6+ months | 3-7 days | Multiple RCTs. 5-year follow-up: NO differences in emotional/behavioral outcomes. |
| Full Extinction (CIO) | 4-6+ months | 2-4 days | Fastest. 5-year follow-up found NO long-term harm. Cortisol normalizes by day 3. |
| Chair Method (Gradual Retreat) | 6+ months | 2-3 weeks | Less studied but clinically used. Gentler approach. |
| Pick Up Put Down | 4-8 months | Variable (weeks) | Limited studies. May overstimulate some babies. |
| No-Cry Methods (Pantley) | Any age | Weeks to months | Minimal formal research. Very gentle, very slow. |
